Notice for Patients Receiving Ongoing Care at Our Clinic for Hypertension, Dyslipidemia, or Diabetes

 As part of the national initiative against lifestyle-related diseases, the Ministry of Health, Labour and Welfare revised the medical fee schedule effective June 1, 2024 (Reiwa 6). Patients who have been receiving care for hypertension, dyslipidemia, or diabetes and have been billed under the "Specific Disease Therapeutic Management Fee" are now required by the government to transition to the "Lifestyle-Related Disease Management Fee" so that more specialized and comprehensive treatment management can be provided.
 
 Patients with comorbid specific diseases such as cerebral infarction, heart disease, bronchial asthma, chronic liver disease, fatty liver, familial hypercholesterolemia, or chronic gastritis will continue to be billed under the "Specific Disease Therapeutic Management Fee" as before, with no changes

 The "Lifestyle-Related Disease Management Fee" is intended to provide more specialized and comprehensive treatment management based on an individualized care plan. Specifically, a government-designated "Lifestyle-Related Disease Care Plan" will be prepared periodically and provided to the patient.

- Goal setting tailored to each individual
- Specific guidance content (diet, exercise, alcohol, smoking, medication use, etc.)
- Test results and information needed for treatment (blood pressure, blood test data, body weight, etc.)
These items will be explained to the patient and entered through consultation.

 When the care plan is first prepared, the patient's signature is required only once, in accordance with government instructions. We appreciate your cooperation.

 In connection with this revision of the medical fee schedule, the out-of-pocket cost at the time of payment for patients transitioning to the "Lifestyle-Related Disease Management Fee" on or after June 1, 2024 may slightly increase or decrease compared to the previous "Specific Disease Management Fee." We appreciate your understanding.
