• About Your Health Insurance Card

  Please be sure to bring your health insurance card or My Number Card when visiting the clinic.
  Without an insurance card, treatment will be billed at the full self-pay rate, so please be aware.

• Notice of Temporary Closure & Shortened Hours

  Consultation on Saturday, June 13, will end at 15:30.
  The clinic will be closed on Monday, June 29 and Tuesday, June 30.

• Clinic Closures & After-Hours Calls

  The clinic is closed on Wednesdays and Sundays.
  Calls received outside of consultation hours (after 17:00) will be handled by an answering machine.

• Notice for Patients Receiving Ongoing Care for Hypertension, Dyslipidemia, or Diabetes
